I recently started doing letos for the server I play on and somebody asked me to rename a couple characters. Using dummy names here...
Andrea: From the character selector her name is "Andrea Andrea" instead of just "Andrea" but ingame it is fine
Alex Andrea: Name shows up "Alex Alex" in the selector, ingame she is "Alex" and Andrea isn't showing up either place.

You're talking about Leto - the standalone program, right? Have you tried Advanced Editor?
Open your character file in Leto. Go to Tools -> Advanced Editor. The first two fields are for characters first and last name. Change them and save the character from Advanced Editor (right click -> save). Exit 'normal' Leto without saving.

That's what it does when a character doesn't have a last name.
